Implementing a community-based heart failure clinic to optimise the care and holistic management of symptomatic patients with chronic heart failure across Leatherhead Primary Care Network (PCN).

A Joint Working Project between Boehringer Ingelheim Ltd and Leatherhead Primary Care Network, aims to improve the holistic management and optimisation of medicines for complex / co-morbid patients with symptomatic heart failure in primary care.

Leatherhead PCN is a part of Surrey Heartlands Integrated Care System (ICS). Within this ICS, heart failure is a significant burden on the local health system and compared with all other cardiovascular diseases, with more than 2 hospital admissions per heart failure patient in a sample taken over  2021, it is amongst the highest frequency across all cardiovascular diseases1 .

This collaboration will support heart failure patients who have complex conditions, currently waiting long periods for consultation.  The collaboration will identify the most appropriate patients to benefit from review, intervention and optimisation. 

The project aims to operationalise in early 2024 and is anticipated to run for 12 months. An evaluation will be produced from data captured over the duration of the project.

Boehringer Ingelheim Ltd is pleased to collaborate with Leatherhead Primary Care Network to design and deliver a new model of care which will provide innovative approaches to the treatment of heart failure patients in primary care.
