A shared care pathway to bring care closer to home for people living with interstitial lung disease at Papworth Royal Papworth Hospital NHS Foundation Trust and across the east of England. A partnership with the Royal Papworth Hospital NHS Foundation Trust and East Suffolk and North Essex NHS Foundation Trust.

A pharmacist-led service for people living with type 2 diabetes in East Belfast to optimise their care, while upskilling healthcare professionals in diabetes management. A partnership with the Federation of Family Practices East Belfast.

A discover project to gather insights on chronic kidney disease provision which identifies new solutions to improve the health and wellbeing of people in Greater Manchester. A partnership with Health Innovation Manchester.

Implementing a new cardio-metabolic care pathway for people living with type 2 diabetes following acute coronary syndrome and supporting continuity of care across Lincolnshire. A partnership with the United Lincolnshire Hospitals NHS Trust.

Re-designing a care pathway for people living with heart failure in Widnes and addressing population health needs across Cheshire and Merseyside. A partnership with the Cheshire and Merseyside Health and Care Partnership.
